Help a Second Noob

I have a table in Access that currently exists as left (of attached image) and I am wanting to design a query (hopefully) that can transform the data to a new table and arranged as right (of attached image). Could anyone let me know how this can be done? UPDATE: accdb file is attached.

Explore CROSSTAB query. Access has a design wizard.

You can explore using a power query with transform in Excel to effectively normalise the data assuming this is a one off exercise.

Minty, think data is already normalized. It is the first 3 columns in image. Rest of image is desired output. OP used Excel to produce this mockup but data in Access.
