Is there easy way to create columnar form with header on the top of the text box for about 30 fields

I have a table with 40 columns that I would like to show in a columnar format with field name on the top of the text box instead of left side of the box.
Actually I am trying to mimic my Excel survey form which has grid values of 5 rows with 15 columns in one sections and 4 rows with 5 columns in second section for each unit(ID). Is that possible can you share some example forms or code?

    Thank you,

Reports can print left to right, but
Databases want to print data top/down. Not left/right. (thats Excel)
Use the tools given that make it very simple to show the data. (top/down)

Sounds to me like something that could only be done with 2 subforms side by side, on a main form. However, to get a specific number of records (rows) if that's what is being asked for would be more challenging if there are not enough or too many records to provide that exact row count. It sounds like a job for Excel.
