Hard to help without knowing your data structure are the 2 tables related?

More info might help as noted, especially as to whether or not this is for a new record or edit. .
The word 'autopopulate' could mean you are inserting default values in a new record. Could mean that you want to filter the form's existing records based on those values.
Are the 'couple of unique fields' going to be combo selections or not?

its with existing data table on contains data about clinical trials like the trial name , the pharma company, key trial personal and a unique reference number that is specific to the trial. The second table contains the details of the patients in the cancer study, the two tables are linked with the same unique reference number. The form i have for updationg and editing recordes uses fields from both tables. When i type in the refrence number i want some fields appear on the form from each table so i dont have to type in all the details again and again.

What you need to do is create 2 forms 1 for each table. I'm assuming that the link between the 2 tables is a 1 to many join the first table being the main table with regards to a specific trial. The second is a demographic table for the patients within the trial. So your trial table would be the main form and the demographic would be a subform with a link between the forms on the unique reference number. For further help research forms/subforms online. Also you could create a sample access database with the 2 tables but remove the data and post that here.

I don't agree. After re-reading this, I think the OP wants to pre-populate some controls with information from one record when creating a new record although the post doesn't explicitly state this is about creating new records. Nor does it really answer my questions on record edits/new nor default values.

I'm not sure we have a one to many relationship since I suspect a one patient to one trial relationship, and a form/sub form is for a one to many relationship. For me, until it's clear what is going on I think that I can't help further. Sorry.

Micron, I couldn't agree more. the OP has not provided enough information to give a definitive answer. I made some assumptions when writing my earlier post but as you pointed out that only works if my assumptions were correct and I was 50/50 on that at the time.
