I would suggest learning the basics of Access first. There are many online resources for this. To answer your question to build a basic form. You can use the form wizard, which will allow you to select your record source, fields, and format you want the form in.

    why Access delete my zeros

I imported a table from excel that have 500 records, I want to used the field name "BeerItem" as primary key, because I have unique data. but when If use data type "number" erase the correct beer item such 01152 it changes to 1152. (I am using TEXT data type in my main table) But when I tried to used text data type with "beeritem" field in another table does not allow me to use zeros in the beginning of the number. Can someone tell me what I am doing wrong. please give me suggestions. thanks

You could change it to a numeric data type and select double as the field size and type in something like 00000 in the format field. This will add leading zeros for any entries that are under 5 characters in length. You can enter more 0s if need be if you're going to have entries that will be more than that.
