Print Records with Yes only

I am looking to design a form or report where I have 30-40 different fields and each field has a check box y/n against it, and I want to print the form/ report for only those fields with have Y against them and don't print where I choose N.

I really appreciate if anyone could help me to solve this problem.

You want to dynamically build report structure? This is not practical. Suggest you build a standard set of reports and users select the report they want, not which fields.
