Data Entry form with prepopulated fields

I have have form that has a combobox that prepopulates two other fields. Then I have data entry fields below that to add additional information. How can I get the form to prepopulate the fields but then once the new data has been entered make it create a new record, instead of adding data to the record associated with the combobox?

Huh? What do you mean by 'create a new record, instead of adding data to the record associated with the combobox'? Do you just need to move to new record row?
