What do you mean by 'with checkboxes'?

I would like to create a customer survey and email it out via External Data >> Create E-Mail. When I run through the wizard and compiles the email, all Yes/No check boxes are converted to fields the user have to type in, i.e. the check boxes are text boxes.

I don't have much experience with the Collect Data utility but I think the fact that it converted the checkboxes to textboxes basically answers your question. As with most wizards, don't have a lot of control over what is generated.

The textboxes allow anything as entry, not just Y or N?

depending on your setup, you have to type either Yes/NO or True/False. would there be away of creating your own form and send it via email to collect data, without using the wizard?

It would not be a user fill-in form email unless HTML code could be used to construct things like checkboxes and textboxes within the email body. I know HTML code tags can be used to format text within email body, beyond that no idea.
And code to connect to Outlook folder and extract data from email would be complex.
