    Email Form displays all fields as text fields to be typed

    Hi guys,

    I am sure this is an easy question, but after days of searching, I am yet to find the solution.

    I am creating a Customer Satisfaction Survey to be sent via Outlook, and would like to add check boxes etc., but have found all my controls show as text fields, instead of check boxes.



    Can one have check boxes in an email form? If so how do I go about creating it?

    I would appreciate it if you could point me to a post or solution!
