    New User - need to create an appointment scheduler

    I just started working for a Nursing Home/Assisted Living Facility and I schedule transportation. I need a database where I can enter the following information: Resident Name, Resident Address, Resident Phone #, Appointment Date, Appointment Time, Dr. Name, Dr. Address, Dr. City, Dr. Phone # and I need to make it where it is easy to enter the data and then create a report for each week that will print out the appointments with the driver I have assigned to transport the resident. We are a non-profit and I am trying to create this myself to save money. I am now pulling my hair out. I am wondering if anybody may know where I can get some sample database files that may help me out here. Any assistance is appreciated.

    I have been utilizing the power of google... I found, nothing much. I'd be willing to donate some time into this for you, if you have anything started, as rough draft as it might be.. post it here and I will poke around and see what I can come up with.

    Have you ever considered a shared calendar? It would not have a nifty form... but it would provide other benefits.. such has techs or drivers being able to keep up to date via their phone. Here is a test calendar I created. I marked this one public, but I could invite specific people and give them specific rights.

    A database is definitely possible, but this might be a viable option as well.
