File has been attached, zipped, repaired and compacted before uploading. What is attached is a stripped down, dummy version of the original database. The names have been changed to maintain privacy.

The 3 checks boxes in question are labeled: Active, Interim and Final.

I don't think your problem is limited to the check boxes. The query is read-only, so the form is too.

Thank you for the feedback. I don't know how the query became read-only, so I don't have a clue to undo that. Any suggestions on where to start?

Edit: I fixed it (or rather, pbaldy fixed it by nailing the source of the problem). I had an additional table on the query that I ended up deciding not to use. I deleted that table. Now the query, and thus the form, work. I'm not sure why it worked -- it's one of those problems you never encounter in class.
