Button opens form differently than double-click

I have a form that opens quite normally when I double-click (see Pic 1).



However, when I use a button to open it, the middle sort of goes away both in "Normal" view (Pic 2) and (preferred) "Dialog" view (Pic 3).



Pic 4 shows the button macro.



Thanks for any thoughts you might have on how best I can resolve Pic 3 to be like Pic 1 when the button is pressed!

Change the Data Mode argument. You've told it you want the form opened to add new records only.
