prevent diplicate record

I have databse with two form.
mainform+subform

main form with shopname
and sub form shop_phone,address

when data entry prevent diplicate and view all info with box massge diplay to me.
I need code for above function

In my sample you can not make duplicates in the phone nuumber, is this what your looking for?

when I enter in form shop name and phone number, i need quick search
if that record is fund massage box appear tell this info been found do u want to view it?Yes or No
when i click Yes it show all detail info,

that all i need see attched file sample
