how to add categories and sub categories in subform

So what is wrong with what I have suggested based on MajP's example??

The fact the it should be a continuous form and not a datasheet. Surely either would suffice?

I get picked up on several things I post here and elsewhere, so no need to take it personal.

Would you not relate the injury stuff to the SUBcategory and just use the Category to filter the Subcategories? Each subcategory should belong to exactly one category, so storing category at all would be redundant.
