    Question Table Fields and Sets of Records

    Hello All! First post.



    I have started creating a relational database. It is based off a real physical form. I have created all the questions on the form in one table. The questions are longer than 255 characters and had to be set to Memo. I would like to utilize this database as a way to fill out the form and save the answers for each customer based on Name and Date. Name is to separate from all the different customers and Date because a customer may fill it out several times. How do I link the two concepts together? I do use indexed primary keys which are unique numbers (some are autonumbers and some are based on a unique value).

    I hope that I didn't confuse anyone. Thank you for your time.

    Beautiful. I was looking for something like that in a few Access books. These two definitely helped me organize my thoughts, as well.
