Transmittal Form

Good day!

I am writing to ask you help and support regarding transmittal form. Considering your expertise and experience in this matter, I believe you would be the best person to assist me. I am currently tasked with preparing a transmittal form using Microsoft Access. Please see below sample. I am very much thankful for your assistance and support.

Access is a program for building databases and maintaining the data contained therein. So everything starts with designing a database (tables, fields, relationships). Later, you create forms (to enter and display data) and reports (to print data). So you never start with a form (by the way, I suspect you mean a report).

So my question is: do you already have a database? And if so, what does it look like? If not, you will not be able to create a form or report.
