    Hi Shaun

On your Main Data Input Form would you want your process be as follows:-

    1. For a specific Student you would set the Goals and Objectives
    2. Then for each specific Objective you want to go through all the various Interventions specifying Start Dates , Start Time & End Time etc...??
You can PM me if you need further help.

Here you go. This is not every objective and every intervention but it's a pretty good list. They are specific to the student, although most of the interventions are the same. Some students will have interventions that are specific to them

When you state main data input form, do you mean the progress notes?

There are two programs, HEIGHTS and PREP, which use different types of progress notes (I hadn't gotten that far in the access that I sent in the beginning)

    Heights notes are as follows:

Specific student, MAID, DOB, date, start time, end time, units (units are 15 minute increments determined by the start and end time), class, type of service (
Assessment, Collaterol, Family,Group Intervention, Group Session, Individual, Intervention, Non-billable Contact, Tx Plan Devep), Purpose (short text field), measurable objectives from student's treatment plan, and what interventions were used during that time, the effectiveness of each of those interventions (Yes, No, Mixed Results, Other), the Response (long text field), and Plan (short text field).

    PREP:

Specific student, MAID, DOB, date, start time, end time, units (units are 15 minute increments determined by the start and end time), class, type of service (Assessment, Collaterol, Family,Group Intervention, Group Session, Individual, Intervention, Non-billable Contact, Tx Plan Devep), measurable objectives from student's treatment plan, what interventions were used during that time, the effectiveness of each of those interventions (Yes, No, Mixed Results, Other).

The a separate section for that day that has a daily summary

I'm doing all of this to solve the problem I'm having with the current database that is not doing the treatment plan and progress notes correctly

    Hi Shaun

The zip you sent does not contain the actual BE database.

You need to also close out of the Db before zipping it
You can PM me if you need further help.
