    Survey form making

    Sir/Madam,

i need my form to map the code to the details in the database. for example when i enter 1 in text box it should write "Excellent" in database table similarly for the codes 2 through 5 as "Good", "Satisfactory", "Un-satisfactory" and "Poor". how to get this in database?

now,the problem is how to make link between textbox2 to the table?
Macro under lost focus/ got focus produces error...
    Is there any alternative option?

    Thanks in advance..

Did you notice the part at the end where you would set the value of the second textbox in code and have it be bound to the table?

You might want to look at "At Your Survey" by Duane Hookom.

"At Your Survey (AYS) is a full featured application that allows users to create their own surveys by designing the questions and providing a lookup of possible responses"
