Survey form making

Sir/Madam,


i need my form to map the code to the details in the database. for example when i enter 1 in text box it should write "Excellent" in database table similarly for the codes 2 through 5 as "Good", "Satisfactory", "Un-satisfactory" and "Poor". how to get this in database?

Still my problem is not solved. i could make the value corresponding to the code entered in textbox1 to textbox2 by using Dlookup function in control source property.
now,the problem is how to make link between textbox2 to the table?
Macro under lost focus/ got focus produces error...
Is there any alternative option?

Did you notice the part at the end where you would set the value of the second textbox in code and have it be bound to the table?
