Query problem

I have a table with 19 variables as follow,
Each variable was rated with scale as indicated in bracket.

1.) DEPRESSED MOOD (0-4)
2.) FEELINGS OF GUILT (0-4)
3.) SUICIDE (0-4)
4.) INITIAL INSOMNIA (EARLY) (0-2)
5.) INSOMNIA MIDDLE (0-2)
6) TERMINAL INSOMNIA (LATE) (0-2)
6a.) HYPERSOMNIA (0-2)
7. WORK AND ACTIVITIES (0-4)
8) RETARDATION (0-4)
9) AGITATION (0-4)
10) ANXIETY PSYCHIC (0-4)
11.) ANXIETY SOMATIC (0-4)
12) SOMATIC SYMPTOMS GASTROINTESTINAL (0-2)
13) SOMATIC SYMPTOMS GENERAL (0-2)
14) GENITAL SYMPTOMS (0-2)


15) HYPOCHONDRIASIS (0-4)
16) LOSS OF WEIGHT (0-3)
16a.) WEIGHT GAIN (0 -3)
17.) INSIGHT (0-2)

I would like to add the total together for all the items but 6a and 16a will only be rated if qu’s 6 and 16 are rated 0.

So there are total of 17 variables will be sum up and 2 of them (Q6a & 16a) were determined by sub question (Q6 & 16) if it equals 0.

Can someone help me how to make this query?

Since 6 and 6a will not have number in both, just simply add all fields together and you got it.
