Update the status of a record on a table based on which status checkbox is selected in a form

I have a table that contains 4 Status checkboxes labeled Ready for J8 Review, Rework, Rework Ready for J8 Review and Reconciled and I need to update the table anytime the status is changed on a form for each record. Right now, if a status on a form is changed from J8 Review to Rework and I view the table, both checkboxes will be checked. How do I get the table to display a checkbox from the most recent status update only.

Instead of a yes/no type for the 4 status fields, date would be more informative.

It would be simpler to have a single status field with 4 values to choose.
