Recording Test Scores and Radar Charts

I am trying to add a feature to record test scores then show these as a radar chart for an individual.

I need to record the score for each question (the answer is a number between 0-9), the date of the test and the person taking the test. So far that seem easy.

However, I want to show the scores as a radar chart and show improvement. Alas this is where I come unstuck.

How do I show the question (they're always the same questions) to create the categories in the radar chart? Which table do I record these in?

My results table includes fields for ID, name, date, Q1, Q2...Q9.

I have made some progress. The data is now kept in a table with ID, PersonID, Date of exam, Question (from lookup), Score (number). This allows me to see the results on a pradar chart and pivot table.

However, inputting the data is very clunky. The input form only shows one question and one score text box. This requires many actions to input the data.

Can I add all the data from one form? There are currently 9 questions and each requires an answer.
