Explain more - a LOT more.
Record only the document id's and related info? Retrieve files from a network based on db references? Store the documents in the db? Manage approvals? Manage review prompts?
    etc, etc,...

I already a made one table of the database. It will be my document monitoring if its approved, checking the status, manage the revisions and making transmittals. and i found a image in google that gives me some idea of the database i want to build.

Based on what you've posted, IMHO you're not ready to build anything yet. You don't want to put everything in one table, use reserved words, have spaces in field names, and you're doing all of that. Here's some links I often suggest for these aspects if you want to improve your chances
Normalization is paramount. Diagramming maybe not so much for some people.

For what it's worth, I have a complete document AND record management db that I developed in a partnership, but you didn't answer all of my questions so I have little idea if it would be of any use to you.
