creating a switchboard, incorrect display on sub menu

I've created a switchboard for my agency's database so that non-pc people can access parts of the DB. On one of my switchboard pages--the form displays one of the items on the list--not the title I gave it "FORMS". I've gone through switchboard manager and I'm fairly certain that I created this correctly. i've attached screenshots of the successive pages both what the end user sees and the information I put in. You are starting on the default Switchboard ADI. The pages i dont think are browsable in that view. You would need to make links to those pages in the contained pages.
