This is a simple example to reserve dates sick and you design tables patient table and table reservations and you link between the two tables ONE TO MANY
But sometimes be booked an appointment by telephone, how can register the patient in this case

Don't understand the question. What does it matter that the information is provided in person, over phone, by email, or sticky note? Enter data into table.

I agree with June7! Regardless of how the appointment is made, you have to input the patient information (name, birthdate, gender, etc) for the patient table/form before you can make an appointment for them, in the subform. Whether you take the infomation face-to-face or over the phone doesn't matter.
