Forget Password Form

I am trying to build a form for Forget Password Functionality. Where i want employees to select their name from combo box and hit ok which should send their previous password stored in database to them.

You would have to have a separate table to store their last password apart from the normal password table. When you say send, you mean email? A simple append query would be all that is needed to store their last passwords, but of course if you have a Login form that has a way to change their password, you would have to code it to append the new data.
