Possible question in the design of a database

Dear All
I want to analyze the system and a small clinic you design tables for the patients ... but the question has to sometimes be booked by appointment Telephone Will I do the design table reservations or do I put the main data table bookings for patients immediately with

I would use another table for reservations and use a reservation subform in your main form

I have attached a little database to show what I mean, doing it this way means that you also keep a history of all appointments that patient has made, and you could add fields like failed to keep appointment ect
