• inventory record issue to employee
• job performance
• inventory control of equipment
• forms, queries, tables, login forms

Are you expecting someone to do it all for you? That's unlikely to happen. Most of us are more than happy to help people with specific problems they're having. Doing the project for you is beyond the scope of a free forum like this.

Or you could check the Microsoft office download site for a template that might be close to what you are looking for. Then do modifications to get exactly what you want.
