New Datbase with specific Columner Form Design

I am needing to create a new database, and would like the attached design of a Form for the input. I would like help in determining if we
can in Access, build a form to look similar to the attached. We like the design, but not sure about the columns in an Access form. Each

week we fill out the info for a given week and submit that info. Then the next week we call up the form and fill in the second week on
info and etc. So each week we would like the prior weeks info on the form as a visual. I have no idea how to setup the database and even know
if this is doable. We are basically tracking deposit and checks and expenses in a Trust account for several accounts. Currently we use this
spreadsheet for the month and the data is not being saved, we just delete the data it is saved no where, we now want to keep that data.
In addition if anyone has a template already such as the attached, please let me know. Thank you for any help or guidance on whom can
    help.

    The simple answer is no.

That isn't a data entry structure, it is a report output and it is not in normalized arrangement. Probably a difficult report to produce.
How to attach file: http://www.accessforums.net/showthread.php?t=70301 To provide db: copy, remove confidential data, run compact & repair, zip w/Windows Compression.
