database and checkbox Problem for Survey Program in access 2007

hi Good Day!

I'm having a problem with my project right now,I am making a survey Program Using ACCESS 2007.
Everytime i select in my FormSurvey the problem arise, my tblDatabase isn't saving or updating. And the database shows numbers instead of the value of the comboboxes. And the 2nd Problem is how do i make a questions with having checkboxes as my option/choices wherein if i have to choose from one of the value of that checkboxes it will automatically save to my tbldatabase.
