    Input Forms - How To Input Multiple Fields/Records?

    A case in point is I have an Activities Input Form. It shows the name of the activities, its date, description and ATTENDANCE OF THE MEMBERS.
    All of the previously mentioned, except the ATTENDANCE, requires only one entry. In the attendance, it requires multiple blanks to accompany several members.

    The form would like this:

    Name: Weekly Film Viewing
    Date: April 8, 2013
    Description: Manhattan
    Attendance/Members
    (1) Soren
    (2) Saren


    (3) Ben

    However, I cannot make any multiple entries for the black of Attendance. I only know how to make one. :/
    Need help badly :/

    More context about WHAT you are trying to do please. You know WHAT you're doing and talking about, but the reader does not have much info to offer assistance.

    If it's attendance, there is a data model showing students and classes and a concept of "roster" for attendance in this model,
