Lightbulb Problem with data going to the wrong file

First of all I will let you know that I know enought about Access to be dangerous. Certainly not an expert. I put together a database to track our 500+ cell phones in our company. I have a main table and then two sub tables with forms to tract the history and accessories. this has been working great. All of a sudden when I've added a new user to the main table and then have added to the accessory and history table, the information is being attached to a random user and not saved with the correct file. This doesn't happen with the other names in the database. I've tried to delete and re-add and the same thing happens. I don't know where to look for the problem or how to fix it.

If anyone can help me out, I would really appreciate it. I'm sorry, this probably isn't real clear.
