Need hlep with input form

I have created a form and have 2 fields that are populated with state names. Each records has a different state in both fields. I am trying to find a way to have the user input one state name into one input field in the form and have the results pull on the two different fields. This is my current query.

The need to be on separate lines, which in SQL view translates to OR instead of AND. Since you have another criteria, it would have to be repeated on the line underneath.
