For parameterized query approach (query would be the RecordSource of form), review tutorials at http://datapigtechnologies.com/AccessMain.htm especially 'Build a Custom Filter in Your Form' and 'Creating a Basic Search Form' in Access Forms: Tips and Techniques section.
How to attach file: http://www.accessforums.net/showthread.php?t=70301 To provide db: copy, remove confidential data, run compact & repair, zip w/Windows Compression.

The videos are very specific and have served to teach thousands of beginners. I am not going to try writing a book here in a forum thread on Access basic functionality. I suggest you get an introductory book on Access and go through exercises.

You can also do a google search on 'how to build a search form in Access'. There are many other tutorials and threads discussing the topic. Here is another http://www.everythingaccess.com/tuto...-criteria-form.

The prompt is because Access can't find the control referenced, usually due to misspelling. Do you want to provide project for analysis? Follow instructions at bottom of my post.
