    Duplicate records listed!

I created two tables that are linked. Then I created a form with both tables (the 2nd table is a subreport in the form). There are 17 unique clients in the 1st table and each client has about 20 records listed within the second table (subreport). I ran a query and somehow it messed up my form and now instead of 17 records each with 20 records in the subreport; it lists 204 records. See attachment. I don't know how to make it go back to 17 unique clients in with individual records listed within the second table (subreport).
