for each id in each month

First thing you may watn to do is put your data in the forum into a format that is easier to read for us....it all runs together in your initial post.

Try giving us a screenshot of the data...or export a few records to an excel spreadsheet and posting that. Its hard to determine where one variable begins and another ends.

note in type column more than one information in different dates

like "Annual Leave", "Sick Leave", "OFF" and more

example:
