Table and form creation

Hi there I am trying to create x2 single forms.

The 1 single form is called home score card, however when enter each players name it duplicates itself into the next field, what i want to happen is I want to put each players name into each space with the corrisponding results, and have it update the data in the write field.

Please see attached to see if anyone can do this for me.
