run parameter query based on option box selection

    Hello,

I want to run a single query using parameters from a series of option boxes on a form.

The form is called "Parameter Query" and I want to use option boxes that mimic those from the "questionnaire" to make it easy for whoever needs to run the query.
For example, I need to select "orderobject"s 1-8 from ORDER table where the answer to question10 on QUESTIONNAIRE table is male or (stored as 1) from the option boxes. Eventually I will need to make this query run for as many option boxes as I have questions on the questionnaire.

How might I make a form such as "parameter query" that allows the user to select the option they want and run that query as such?

    I appreciate any help!
