Combining Table Fields

My database is being populated by data being released from scanning software. When inputting the data the date is entered as 3 seperate fields (Year, Month, Day). I have a fourth field in my database for "Date". Is there a way to combine Field Year, Field Month, and Field Day together into Field Date within my Table (YYYY/MM/DD format)?
