Attempting to take multiple lines of data and create a single instance for each unique identifier

My mind has gone completely blank on me and it is beyond frustrating.

I'm trying to do a simple task I have done multiple times before, but I might as well have never used a database based on how empty my thoughts are on this. I'm having trouble even finding the words to describe it.

Essentially, I have a table with multiple ID's (wells). Each well has a line for that months numbers. I have to graph this info in separate visualization software, and also link each well to other data. So essentially, I need a single instance of each well, not a column for every single month it has existed. Problem is, each well has an arbitrary number of rows (dependent on it's life span).

I'm totally lost, and yet, I know I have done this in the past. Some guidance would be extremely helpful.
