Checking Date Range in Multiple Fields

I have a table with 18 fields and 15 of them are date fields (mm/dd/yy format). I want the user to enter a Start Date and End Date on a form. Then I want a query to check the date fields and return any record that has a date field that fits the date range criteria. Thanks for your help.

Would need same filter criteria under each field but on separate line to invoke the OR operator (looks like stair steps).
