This morning came in at 5 am and happy to say resolved the issue.
Turns out the main issue was table/field construction. Lookup Wizard was changing fields to "Number" fields instead of "Text" also changed primary keys to Org ID and allowed duplicate entries.
Finally adjusted Operator query:
[Forms]![frmOpReport]![OperatorCombo] (also forgot to name input boxes on search form).
Report is coming out exactly the way it needs to.

Last (hopefully) challenge is to add a button on the report that will take report and send email through Outlook. Also cosmetic changes.
