Using a form to select criteria for a report

Is there ever a time that you would NOT select Compliance, Non-Compliance, Not-Applicable or (Compliance and Non-Compliance)? ie all three check boxes would be unchecked?

Yes when I just need a blank checklist printed or if I don't care about compliance.

Also I think you may be misunderstanding the compliance & non-compliance part. There will never be more than one compliance field selected at any time for each record it will always be one and only one of the three choices. Where the compliance and non-compliance comes in is if I need a report that shows all records that have been marked either compliance or non-compliance. so it would be like taking two separate reports and combining them into one.
