Can't add new (blank) record in a form

I have 3 forms in my database. Two of them allow me to add a new record by clicking on the yellow icon on the bottom bar. However, in the third form the new record button is greyed out, and I can't add a new record. How is this resolved?

    Thanks,
    Joe

What are the properties for the form with Allow as the first word?
What is the source for that form?

Yep thinking the source on the 3rd form is a query?

The Allow parameters are form view, datasheet view, layout view, additions, deletions, edits, filters. All are YES except datasheet view. This form was derived from a table like all the others, yet it doesn't allow data entry.

Look at the Record Source on the Data tab?

As Welch said, go to Properties and then look at Data tab or All tab. If named right, your tables should start with "tbl" and queries start with "qry".
