combo box with options

Hello,
I have made a combo box with yes or no selection.
I want to give an option to fill other information if selected yes. Example:


Family members? yes/ no (if no nothing to fill) (if yes, I want options to fill in other information such as spouse name, child 1, child 2 etc/)
