    How to combine test item in one row ms report

    Good Day mam/sir

    I have a simple question from a noob.

    I just want to make a summary report of a patient with several test request in a one row.

    On my form i have a header like Patient Name, Examination Request etc. i want to make this in single row per patient.

    Please see my attached image.

    This is not how a database works, however,
    you can make a Crosstab query to VIEW the data your way.

    use the query wizard to build the query with output you want to see.
    add a button or tab control to your form to view either the current (editable) version,
    or the Crosstab version.

    Sorry sir i think i have a mistake on my question. i mean on report i want to combine the test request like on my sample image.

    same id and did on the same day and time.
