Issue with BeforeInsert Event

I do not understand why this is suddenly giving me issues. I have run this one-thousand times in the past without issue.



The images show everything. Essentially when the user adds data to any field (date shown as example) the case number to the right auto populates with desired numbering format (format shown in image 2) due to BeforeInsert event; however, this appears to be experiencing an issue now. I appreciate suggestions.

There is no SQL. This example of what it should do. I resolved the issue, luckily I maintain copies of versions as I go along.
