Nope this is not what I want. I want a form that creates username and password. Example, I already added my username and password to my log in form. Some instances that I want to change my username and password using the change user name and password button, just like that.
What log on form are you using?
Where is the change user name and password button?
Did you create this or are you using the built in User Level Security (ULS)?

Just basic log in form with change username and password. If I want to change my username and password later on. I will just click the change user name button and another dialog box will appear. Enter old password:
Enter new: Username
Enter New: password
Ok / Cancel buttons. Prompt will appear: username and password successfully change. Go back to the log in screen and try to log in. We may able to use the new log in username and password and the old password is deleted. I hope I'm clear enough.

I really need help how to set-up the change username and password form. The conditional formatting and the statements needed for me to create my desired output. Which is the statement above..

I do not think you will need to use any conditional formatting for this. Not sure how it would help.
What is the name of the table that holds the user information?
What are all teh fields in this table?
What controls do you now have on your form?
It might be easier for you to post a sample database. If not, then some screenshots would also help.
