    The image is of the third section first column. I had it spaced out but it didn't hold. So I did 'print screen' and pasted it in the Paint program, cropped and saved the column as 1image.bmp(black), and the other is the injected view, 2image.bmp (white) to be changed as a value changes on the Report.

    Cooked ____to be filled in by employee using a pencil pr pen_____
    Routed Drilled ____to be filled in by employee_____
    1st inspection ____to be filled in by employee_____
    Buff/Flapp ____to be filled in by employee_____


    Assembled ____to be filled in by employee_____
    Final insp ____to be filled in by employee_____

    In following your step by step I have a Table, ImageT, with one field having links to 1image.bmp and 2image.bmp both in the same field and in the same record. From design view I added an Image box.

    Am I good so far?

    Again, I said two Attachment fields with 1 image in each field.

    Spacing didn't hold in textbox? Probably because of non-proportional font. I use Courier New or Consolas or some other non-proportional font to control text alignment.
