I created a blank form. Inserted a header with a label and image and change the background to blue. But when I leave design view and go the form the header is not there.

The header Visible property is set to Yes?

The header doesn't have a visible property. I clicked on the ALL tab and there is no "visible" property.
You have asked me a few times to provide DB for analysis. Is there a fee with that?
Yes it does, at least I see it. Must first click on the header bar. It is the first property on the Format tab.

I assumed we were talking about form header, not page header.
Page header/footer sections on form only show if the form is printed (paper or PDF).

Not possible. That's the way it's built. Use Form header/footer instead.
