How can I get new table and report from this form

Hi every body
as attachment you can find part of my Database
I need to record all of items in "Jointheat" form to new table how can I do that please show me clearly
or send to me in my file
and I want to get a report from my form and it is appreciated if you show me too.

Would this solution be suitable:
1) specify the table as the data source of the form
2) in the form's properties, set Data Entry to Yes
3) in the form's properties, set Allow Additions to Yes
