Usage Tracking Template

Hi, I am looking to set up a simple database that will track the usage, replacement, and life of light bulbs. For Example: I have 24 light bulbs and want to be able to plug in every time I change one of them, so that I can run reports and see how often each particular bulb is changed, how long each current bulb has been in use, etc. Just wondering if anyone knew of a template that was already created that I could manipulate into doing this without having to write all the reports from scratch. Reports are my weakness.
